1.Information we collect
We can collect the following types of information:
1.1. Information you provide
Amika collects information from you when you provide it to us. This happens when you fill out an online form, use the Platform or contact us, for example via our website, applications, emails, SMS, phone or in person. The information you provide to us may include information that personally identifies you ("Personal Information"), such as your name, email address, phone number, or your mailing address. We also collect your credit card information and bank details to process payments for homecare Services. You can also provide us information about a person whose care is managed through the Platform, such as that person's Personal Information, Care Services requirements, special needs, and preferences. You agree not to provide us with information about another person unless you have permission from that person.
As part of the evaluation of the Beneficiary's needs and preferences, we will collect information regarding his state of health, his level of autonomy as well as his personality and interests. In addition, if you want us to manage on your behalf the advance payment for tax credit for home-support for seniors, you will also provide us with the information required to complete TPZ-1029.MD.9 form.
1.2. Information provided to health professionals
You or the Beneficiary can provide information to care professionals ("Amika Caregiver"). Healthcare professionals will use the Platform to access your client profile and communicate with you about care services.
1.3. Information from cookies and similar devices

If you interact with our website or applications on a mobile device, your mobile carrier may automatically send us your device and your hardware identifier, device type, carrier, carrier user number, content of your request, and service statistics basic use of your device on our website or our applications. We do not store this information and use it for any purpose. If you provide it to us or send a message from your mobile device, we can also collect your phone number. Your device may also store information about your use of our website and apps. Please consult your device documentation for local storage management.
1.5. Information based on standard Web protocols
When you access our website via a browser, application or other client, such as other web providers, our servers or those of our provider, may automatically save certain information. This data is stored in the server logs and may include information such as your web request, your interaction with the website, the website you visit on our website, your IP address, the type and language of your browser, the date and time of your request, as well as your cookie data.

-To put you in touch with our Amika Caregivers who have common interests with you;
-To share certain information with Amika Caregivers before / during / after the Services;
-Improve our services and develop new services;
- Conduct statistical, demographic and marketing analyzes, in particular to determine how users interact with the Platform and determine the level of interest for the Platform;
-Customize the products and services for you;
-To contact you about the Platform, Care Services or send you the information you requested;
-To inform you of any changes made to the Platform or your account (update of the profile, availability and others).
Only the Amika Caregivers assigned to the Beneficiary have access to certain information in order to offer a quality and personalized service.
3. Sharing personal information
Amika may share your personal information with:
- Some Amika Caregivers, so that they can communicate and provide the assistance and homecare Services;
-With those involved in managing the care of the recipient, such as family members;
-With persons or organizations that you authorize, including persons you have hired to provide services to you such as doctors, other care providers, or Revenu Quebec as part of advance payments of the tax credit ;
-When we believe in good faith that it is necessary (1) to comply with any applicable law, regulation, legal process or governmental requirement; (2) apply the applicable terms and conditions of our Platform, (3) detect, prevent or deal with any other fraud, security or technical matter or (4) protect against infringement of the rights, property or safety of Amika, partners, users or the public.
Any changes to this policy will be posted on our website, www.SoinsAmika.com/Privacy. We will not use the data we collect in a manner that is materially different from the promises made in the privacy polity version under which it was collected without your consent.
5. Update your personal information
In order to update your customer profile, you can call us or access your profile via the Platform.
You may contact us by emailing us at email@example.com and we will make good faith efforts to give you access to your personal information for the purpose of correcting, modifying or deleting it, unless the law provides otherwise. We may refuse to process applications if it requires disproportionate efforts in relation to the risk of invasion of privacy, if it violates the rights of others or if it is prejudicial to an ongoing legal proceeding or investigation. Due to the way we make backup copies of our data, residual copies of the information may remain in our backup systems. We may also be unable to remove the Personal Information that you submitted during public events, and even after it was removed, and may remain visible in cached and archived locations or on other out-of-the-box websites to our control. We may charge reasonable fees to access your personal information if it would require disproportionate effort. Updating or deleting your personal information will not affect any other services that Amika provides to you.
We recognize that security is important and take reasonable steps to protect your personal information against loss, misuse or unauthorized access, disclosure, alteration or destruction. If you believe that the personal information you have provided to us has been compromised, or if you notice a potential security vulnerability on our Platform, please contact us at admin@SoinsAmika.com.
7. Communicate with us
7.1. Emails and other notifications
You agree that Amika may send you communications relating to your account and Amika's Services. These communications can be done by email, phone, SMS or notifications via applications. You may choose not to receive certain marketing messages from Amika by e-mail by contacting us, but you may not choose not to receive administrative messages relating to your account or legal matters. You agree to receive all legally required notices from Amika by posting them on our website, via the email you provide to us or via the email associated with the Amika Platform.
